WORK WITH US

Join us as we work to connect local care. You can get involved as a Lived Experience Partner to share your experience using our healthcare system.

Priority
Population

We will focus initially on enhancing coordinated care and services for adults 65 years and older who:

  • Live alone or have a poor support network
  • Experience functional decline, mobility challenges, multiple illnesses or cognitive challenges
  • Have been hospitalized within the last 12 months

 

The populations we serve will expand as the work of our team evolves, and at maturity we aim to improve the healthcare experience for all members of our community.


We also heard from our community that further support is needed for people without a primary care provider. As a result we are focusing on a number of initiatives for unattached patients, such as increasing access to preventative cancer screenings..

Patient Centred Approach

By putting the patient at the centre of our system, we aim to reduce confusion, support more efficient navigation between care settings, and help keep people healthier, for longer.

What is an Ontario Health Team?

Ontario Health Teams are being introduced to provide a new way of organizing and delivering care that is more connected to patients and their local communities. Under Ontario Health Teams, health care providers (including hospitals, doctors and home and community care providers) work as one coordinated team – no matter where they provide care.
